First, is a culture that is not willing to change in result of the project. The second is not having reliable sponsorships. The third is not having sufficient answers to problems that need to be solved. The fourth is not having the members of the project group interested. The fifth is a lack of confidence in the project itself. If these can be spotted early and adjusted it is more likely that a project will be successful.
